CARC 183 appeal letter template — Referring Provider Not Eligible to Refer
CARC 183 indicates the payer determined the referring provider listed on the claim is not eligible to make referrals for the billed service. This template helps staff contest the denial when the referring provider's eligibility or identifiers can be documented or clarified.
When to appeal vs. correct & resubmit
Appeal when you have documentation showing the referring provider was eligible, properly enrolled, or correctly identified (e.g., correct NPI, active enrollment status, specialty qualification) at the time of service, and you believe the denial resulted from a data mismatch or payer record error. If the referring provider was genuinely ineligible, not enrolled, or the referral requirement was not met, correct the referral (use an eligible referring provider) and resubmit instead of appealing.

Frequently asked questions
What does CARC 183 mean?
It means the payer determined that the provider listed as the referring provider on the claim is not eligible, under that payer's rules, to make referrals for the billed service.
Should I appeal or correct and resubmit?
If you can document that the referring provider was eligible or that the information submitted was inaccurate due to a data or entry error, an appeal may be appropriate. If the referring provider truly does not meet the payer's eligibility requirements, correct the referral and resubmit with an eligible provider instead.
What documentation helps support this appeal?
Proof of the referring provider's NPI, specialty, and active enrollment or credentialing status with the payer, along with any referral or order records tied to the date of service.
Can this appeal guarantee the claim will be paid?
No. Submitting an appeal does not guarantee payment or reversal of the denial; the payer will review the submission and make its own determination.
