Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Each CPT/HCPCS code is assigned three Relative Value Units (RVUs): physician work, practice expense (PE), and malpractice (MP). These RVUs are each adjusted by the Geographic Practice Cost Index (GPCI) for a specific Medicare locality, then summed and multiplied by the annual Medicare conversion factor to produce a dollar amount. Practice expense RVUs differ between non-facility (e.g., physician office) and facility (e.g., hospital) settings because overhead costs differ, which is why the two amounts can diverge — for this code they happen to be equal. The national amount shown uses standard national GPCIs; actual payment in a specific locality will use that locality's GPCI values instead.
RVUs — work 14.73, practice expense 11.39 (non-facility) / 11.39 (facility), malpractice 3.13 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets the annual deductible; the beneficiary or secondary payer is responsible for the remainder.
- Modifiers, multiple-procedure reductions, bilateral rules, sequestration, and annual budget-neutrality adjustments can change the final paid amount.
- Locality-specific GPCIs will produce a different amount than the national figure shown here.
- Always verify the current, locality-specific allowed amount with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.
Frequently asked
Why are the non-facility and facility amounts the same for this code?
When the practice expense RVU is identical in both settings, the resulting non-facility and facility amounts will also be the same, since work and malpractice RVUs don't vary by setting.
Will I actually be paid this exact amount?
No. The figure shown is a national estimate. Your actual payment depends on your Medicare locality's GPCIs, any applicable modifiers, and other claim-specific adjustments.
How is the conversion factor used in this calculation?
The conversion factor is a dollar multiplier applied to the sum of the GPCI-adjusted RVUs (work, practice expense, and malpractice) to produce the final allowed amount for the year shown.
Does this amount reflect what the patient or secondary insurer owes?
No. The PFS amount is the total allowed amount before the standard Medicare split; Medicare generally covers 80% after the deductible, with the remaining portion owed by the patient or a secondary payer.
