Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare PFS amounts are built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to each code. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, reflecting local cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Non-facility and facility settings can have different practice-expense RVUs, since facility settings assume some costs are borne by the facility rather than the billing provider, which can change the total payment.
RVUs — work 14.83, practice expense 12.86 (non-facility) / 12.86 (facility), malpractice 3.16 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient's deductible is met; the patient or secondary insurer is responsible for the remainder.
- Actual payment can vary due to modifiers, place of service, sequestration, annual budget-neutrality adjustments, and other claim-specific factors.
- Locality-specific GPCIs and conversion factors change annually and can affect the final amount.
- Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC).
Frequently asked
Why does the Medicare allowed amount for this code differ by locality?
Because GPCIs adjust the work, practice expense, and malpractice RVUs to reflect differences in local labor, rent, and liability insurance costs, resulting in different allowed amounts across localities.
Why are non-facility and facility amounts sometimes the same or different?
The practice-expense RVU can differ between settings because facility settings assume the facility absorbs certain overhead costs; when those RVUs are equal, the allowed amounts will also be equal.
Does this amount reflect what the patient will owe?
No. The figure shown is the Medicare-allowed amount before cost-sharing. Patients are generally responsible for 20% coinsurance after the deductible, unless a secondary payer covers it.
Can modifiers change this estimate?
Yes. Modifiers such as bilateral, multiple-procedure, or assistant-at-surgery indicators can increase or decrease the final payment relative to the base PFS amount shown here.
