Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is billed. The adjusted RVUs are summed and multiplied by the annual national conversion factor to produce the allowed amount. Practice expense RVUs can differ between non-facility (e.g., office) and facility (e.g., hospital) settings, which is why two rates are often shown; for some codes, like this one, the practice expense RVU is the same in both settings, so the non-facility and facility amounts may be identical. These RVU, GPCI, and conversion factor values are rendered elsewhere on this page and update each year.

RVUs — work 7.88, practice expense 7.97 (non-facility) / 7.97 (facility), malpractice 1.62 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the Part B deductible is met; the patient or secondary payer is responsible for the rest.
  • Actual payment can vary based on modifiers, place of service, sequestration, and annual budget-neutrality adjustments.
  • Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC).

Frequently asked

Why are the non-facility and facility amounts the same for this code?

When the practice expense RVU is identical in both settings, the calculated PFS amount comes out the same for non-facility and facility claims.

Does this amount reflect what Medicare will actually pay?

No. It reflects the Medicare allowed amount before the standard 80/20 split, deductible status, and any modifier or sequestration adjustments are applied.

Why might my local Medicare payment differ from the national amount shown?

The PFS formula applies Geographic Practice Cost Indices (GPCIs) specific to each locality, so the same code can price differently depending on where the service is furnished.

Will this amount change next year?

Yes. RVUs, GPCIs, and the conversion factor are updated annually (and sometimes more often), which can change the calculated fee even if the code itself doesn't change.
