Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare PFS amounts are built from three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Non-facility (e.g., office) and facility (e.g., hospital) settings often have different practice-expense RVUs, reflecting whether the physician or the facility bears the overhead cost, which is why the two amounts can differ.

RVUs — work 20.09, practice expense 12 (non-facility) / 12 (facility), malpractice 4.62 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient meets their Part B deductible; the patient or secondary payer is responsible for the remainder.
  • Modifiers, place of service, sequestration, and annual budget-neutrality adjustments can change the final paid amount.
  • Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing.

Frequently asked

Why do the facility and non-facility amounts differ for some codes?

They can differ because the practice-expense RVU component accounts for who incurs the overhead cost — the physician's office or the facility. When a service is performed in a facility, some overhead costs are billed separately by the facility, which can lower the physician's practice-expense RVU and therefore the physician payment.

Does this amount reflect what the patient will owe?

No. The figure shown is the Medicare-allowed amount before cost-sharing. Patient responsibility depends on deductible status, coinsurance, and any secondary insurance.

Why might my MAC pay a different amount than shown here?

Locality-specific GPCIs, applied modifiers, sequestration reductions, and annual budget-neutrality adjustments can all cause the actual paid amount to differ from a generic estimate.

Is this the global fee including all related services?

The PFS amount reflects the RVU-based valuation for this code only; it does not account for bundling, global surgical periods, or other claims-specific adjustments that may apply.
