Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's Physician Fee Schedule amount is built from three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, to reflect local cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor (a dollar amount set each year) to produce the allowed amount. Practice-expense RVUs — and therefore the total allowed amount — can differ between non-facility settings (e.g., physician office) and facility settings (e.g., hospital), since overhead costs differ.

RVUs — work 18.53, practice expense 12.1 (non-facility) / 12.1 (facility), malpractice 3.75 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This amount is an estimate only, based on national RVUs, GPCIs, and the current conversion factor — not a guarantee of payment.
  • Medicare typically pays 80% of the approved amount after the patient's annual deductible is met; the patient or secondary insurer is responsible for the remainder.
  • Actual payment can vary by locality, applicable modifiers, claims-based adjustments, sequestration, and annual budget-neutrality changes to RVUs or the conversion factor.
  • Always verify the final allowed amount with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.

Frequently asked

Why does the allowed amount differ between non-facility and facility settings?

The practice-expense RVU component is usually lower in a facility setting because the facility (not the physician's practice) incurs certain overhead costs, such as equipment and supplies.

Why might my locality's payment differ from the national amount shown here?

Each locality has its own Geographic Practice Cost Indices (GPCIs) for work, practice expense, and malpractice, which adjust the RVUs to reflect local cost-of-living and practice cost differences.

Does this page tell me what the patient or secondary payer owes?

No. This page shows the estimated Medicare-allowed amount. Patient responsibility (deductible, 20% coinsurance) and any secondary insurance payment are calculated separately.

Can modifiers change this amount?

Yes. Modifiers (e.g., bilateral, multiple procedure, assistant surgeon) can increase or decrease the allowed amount from the base fee schedule rate shown here.
