Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

The Medicare PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is billed. The geographically adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Non-facility and facility settings can use different practice-expense RVUs, reflecting whether overhead costs (e.g., supplies, staff) are borne by the billing practice or by a separate facility; for this code those practice-expense RVU values are the same, so the non-facility and facility amounts shown are equal. All RVU, GPCI, and conversion factor values used in this calculation are provided separately on this page.

RVUs — work 16.01, practice expense 5.32 (non-facility) / 5.32 (facility), malpractice 4.87 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment — actual reimbursement depends on claim-specific factors.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the beneficiary or secondary payer is responsible for the remainder.
  • Modifiers (e.g., bilateral, multiple procedure, assistant surgeon), sequestration, and annual budget-neutrality adjustments can change the final paid amount.
  • Local Medicare Administrative Contractor (MAC) policies and annual PFS updates may affect pricing — verify current rates with your MAC before billing.

Frequently asked

Why are the non-facility and facility amounts the same for this code?

For CPT 22844, the practice-expense RVU used in the facility setting is identical to the one used in the non-facility setting, so the calculated allowed amounts come out equal.

Does this amount reflect what my practice will actually collect?

No. It reflects the Medicare-allowed amount before patient responsibility, secondary payer adjustments, modifiers, or sequestration are applied.

Why does the amount change by locality?

Medicare adjusts work, practice expense, and malpractice RVUs using locality-specific GPCIs to account for regional cost-of-practice differences before applying the conversion factor.

Will this rate change during the year?

PFS rates are typically set annually, but budget-neutrality adjustments, legislative action, or MAC-level corrections can occasionally alter rates mid-year.
