Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is furnished. The geographically adjusted RVUs are summed and multiplied by the annual national conversion factor to produce the allowed amount. Practice-expense RVUs differ between non-facility (e.g., office) and facility (e.g., hospital) settings, since overhead costs differ, which is why this code may show two different amounts. All figures update annually and can shift due to rule changes or statutory adjustments.
RVUs — work 10.94, practice expense 9.42 (non-facility) / 9.42 (facility), malpractice 3.34 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary payer is responsible for the remainder.
- Actual payment can vary by locality, applicable modifiers, multiple-procedure or bilateral adjustments, sequestration, and annual budget-neutrality changes.
- Always confirm final allowed amounts and coverage policy with your Medicare Administrative Contractor (MAC).
Frequently asked
Why don't you show the CPT code description?
CPT code descriptors are copyrighted and licensed by the American Medical Association, so we cannot reproduce them here. You can look up the official descriptor in your CPT codebook or AMA-licensed software.
Why is the facility amount different from the non-facility amount?
The two settings use different practice-expense RVUs because overhead costs (staff, equipment, supplies) are assumed to be lower when the procedure is performed in a facility like a hospital, where the facility itself bears some of those costs.
Why might my actual payment differ from the national amount shown?
The national amount uses an average GPCI; your locality's specific GPCIs will adjust the work, practice-expense, and malpractice RVUs up or down based on local cost differences.
Does this amount include the patient's share?
No. The figure shown is the full Medicare-allowed amount. Medicare generally pays 80% of this after the deductible, with the remaining 20% typically owed by the patient or a secondary payer.
