Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare PFS amounts are built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to each code. Each RVU component is adjusted by the Geographic Practice Cost Index (GPCI) values for the specific locality to reflect regional cost differences. The adjusted RVUs are summed and multiplied by the annual national conversion factor to produce a dollar amount. Practice-expense RVUs differ between non-facility and facility settings because overhead costs (equipment, supplies, staff time) are assumed to be lower when a procedure is performed in a hospital or other facility that bears some of those costs itself.

RVUs — work 7.7, practice expense 43.93 (non-facility) / 2.61 (facility), malpractice 1.16 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary payer is responsible for the remainder.
  • Modifiers, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
  • Locality-specific GPCI values mean the real amount paid may differ from the national figure shown here.
  • Always confirm final allowed and paid amounts with your Medicare Administrative Contractor (MAC) or remittance advice.

Frequently asked

Why is the facility amount lower than the non-facility amount?

The facility rate uses a lower practice-expense RVU because the hospital or facility — not the physician's office — absorbs most of the overhead costs (equipment, supplies, staff) for the procedure.

Why does the amount change by locality?

Each Medicare locality has its own Geographic Practice Cost Index (GPCI) values for work, practice expense, and malpractice, which adjust the RVUs to reflect local cost-of-living and practice cost differences.

Does this figure include patient cost-sharing?

No. The amount shown is the Medicare-allowed amount before the standard 80/20 split and any unmet deductible are applied.

Will this be the exact amount I get paid?

Not necessarily. Modifiers, secondary procedures, sequestration, and annual budget-neutrality adjustments can all change the final paid amount from the base PFS calculation shown here.
