Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 4 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three relative value units (RVUs) — physician work, practice expense, and malpractice — that reflect the resources typically needed to furnish a service. Each RVU component is adjusted by geographic practice cost indices (GPCIs) specific to the locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Practice-expense RVUs often differ between non-facility (e.g., physician office) and facility (e.g., hospital) settings because overhead costs differ, which is why the two amounts can diverge. The conversion factor is updated annually and applied uniformly nationwide, while GPCIs vary allowed amounts by locality.
RVUs — work 5.88, practice expense 1.93 (non-facility) / 1.93 (facility), malpractice 1.76 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets their deductible; the patient or secondary insurer is responsible for the rest.
- Modifiers, place-of-service codes, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
- Amounts shown are based on national or locality-level inputs and may not match your specific Medicare Administrative Contractor (MAC) determination.
- Always verify current rates and coverage policy with your MAC before billing or appealing a claim.
Frequently asked
Why is the non-facility amount different from the facility amount?
The two settings use different practice-expense RVUs because overhead costs (equipment, staff, supplies) differ when a service is furnished in a physician office versus a hospital or facility setting.
Why does the allowed amount change by locality?
Medicare applies Geographic Practice Cost Indices (GPCIs) to each RVU component to reflect regional differences in wages, rents, and malpractice costs, so the same code can have different allowed amounts in different areas.
Does this amount reflect what the patient actually pays?
No. The figure shown is the Medicare-allowed amount. Patients generally owe 20% coinsurance after the deductible, and secondary insurance or supplemental coverage may affect the final out-of-pocket cost.
Will this amount stay the same next year?
No. The conversion factor, RVUs, and GPCIs are reviewed and can change annually, and budget-neutrality adjustments may also shift allowed amounts year to year.
