Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to the code. Each RVU component is adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is performed, to reflect local cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Practice expense RVUs differ between non-facility (e.g., office) and facility (e.g., hospital) settings, which is why the two allowed amounts differ.

RVUs — work 10.73, practice expense 48.58 (non-facility) / 20.79 (facility), malpractice 1.61 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the annual deductible is met; the beneficiary or secondary payer is responsible for the remainder.
  • Modifiers, place-of-service rules, multiple-procedure reductions, and sequestration can change the actual amount paid.
  • Conversion factors and RVUs are updated annually and may be revised for budget neutrality.
  • Always confirm current rates and locality-specific adjustments with your Medicare Administrative Contractor (MAC).

Frequently asked

Why does the facility amount differ from the non-facility amount?

The practice expense RVU is lower in the facility setting because the facility (e.g., hospital) absorbs certain overhead costs separately, while non-facility settings like a physician's office bear those costs directly.

Why does the reimbursement amount vary by locality?

Medicare applies Geographic Practice Cost Indices (GPCIs) to adjust for regional differences in labor, rent, and malpractice insurance costs, so the same code can pay differently depending on where the service is billed.

Does this page reflect what my practice will actually be paid?

No. This is a reference estimate based on standard PFS methodology. Actual payment depends on your specific MAC, contract terms, applied modifiers, and any sequestration or policy adjustments in effect.

Where can I find the official CPT descriptor for this code?

CPT code descriptors are copyrighted by the American Medical Association. Consult your CPT codebook or AMA-licensed coding software for the official description.
