Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to the code each year. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, reflecting local cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce a dollar amount. Practice expense RVUs differ between non-facility and facility settings, which is why the same code typically has two different allowed amounts depending on where it's billed.
RVUs — work 6.1, practice expense 15.76 (non-facility) / 10.18 (facility), malpractice 0.69 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets their deductible; the patient or secondary payer is responsible for the rest.
- Actual payment can vary by locality, applicable modifiers, claim-specific adjustments, and sequestration.
- Annual updates to RVUs, GPCIs, and the conversion factor — including budget-neutrality adjustments — can change this amount each year.
- Verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing.
Frequently asked
Why does CPT 21445 show two different national amounts?
The non-facility amount applies when the service is performed in a setting like a physician's office, while the facility amount applies in settings like a hospital, where Medicare separately reimburses the facility for overhead. Practice expense RVUs differ between the two, producing different totals.
Why isn't the CPT code description shown on this page?
CPT code descriptors are copyrighted and licensed by the American Medical Association, so they cannot be reproduced here. You can look up the official descriptor in your CPT codebook or AMA-licensed coding software.
Will the amount shown match what I actually get paid?
Not necessarily. The figure is a national or locality-adjusted estimate based on standard PFS inputs. Your actual payment can differ due to local GPCI values, modifiers, secondary payers, sequestration, and other claim-specific factors.
Does this amount change every year?
Yes. CMS updates RVUs, GPCIs, and the conversion factor annually, and these updates — along with budget-neutrality requirements — can raise or lower the allowed amount for a given code from year to year.
