Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) assigned to the code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is furnished, then the adjusted RVUs are summed and multiplied by the annual national conversion factor to produce a dollar amount. Non-facility (e.g., office) and facility (e.g., hospital) settings use different practice-expense RVUs because overhead costs differ, which is why the two allowed amounts for this code are not the same. These calculations are updated annually and can change due to RVU revaluation, GPCI updates, or conversion factor changes.
RVUs — work 21.92, practice expense 23.8 (non-facility) / 7.22 (facility), malpractice 2.5 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the beneficiary or secondary payer is responsible for the remainder.
- Modifiers, place of service, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can all change the actual paid amount.
- Locality-specific GPCIs mean the amount for your practice location may differ from the national figure shown.
- Always confirm the current, locality-specific allowed amount with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.
Frequently asked
Why is the facility amount lower than the non-facility amount for this code?
The facility rate uses a lower practice-expense RVU because Medicare assumes the facility (e.g., hospital) is absorbing some of the overhead costs, such as equipment and staff, that a physician's office would otherwise bear.
Does this amount represent what Medicare will actually pay?
No. It's the Medicare-allowed amount before the standard 80/20 split and before any deductible, modifier, or sequestration adjustments are applied.
Why might my locality's amount differ from the national figure?
Medicare adjusts the RVU components using Geographic Practice Cost Indices (GPCIs) that reflect local costs for labor, rent, and malpractice insurance, so the same code can price differently in different areas.
Will this amount stay the same next year?
Not necessarily. The conversion factor, RVUs, and GPCIs are all subject to annual updates and budget-neutrality adjustments from CMS, which can raise or lower the allowed amount.
