Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare PFS amounts are built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed. These geographically adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Practice expense RVUs differ between non-facility settings (e.g., physician office) and facility settings (e.g., hospital, ASC), which is why two amounts are shown. Work and malpractice RVUs generally stay the same across settings, while the facility practice-expense RVU is typically lower because some overhead costs are covered by the facility itself.
RVUs — work 3.26, practice expense 7.8 (non-facility) / 3.62 (facility), malpractice 0.43 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the beneficiary or secondary payer is responsible for the remainder.
- Modifiers, place of service, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
- Locality-specific GPCIs mean the final amount can differ from the national figure shown here.
- Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC).
Frequently asked
Why are there two different amounts for this code?
One reflects the non-facility (e.g., office) setting and the other the facility (e.g., hospital) setting, because practice expense RVUs — and therefore the total allowed amount — differ based on where the service is performed.
Does this amount reflect what the practice will actually be paid?
No. It's an estimate based on national RVUs and the conversion factor; the actual Medicare payment depends on locality GPCIs, applicable modifiers, and other adjustments like sequestration.
Why isn't the CPT code description shown here?
CPT code descriptors are copyrighted and licensed by the American Medical Association, so they cannot be reproduced on this page.
How often do these amounts change?
Medicare updates the conversion factor and RVUs annually, and GPCIs can also be revised, so allowed amounts may change each calendar year.
