Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to each code. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is billed, reflecting local cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce a dollar amount. Non-facility and facility rates differ mainly in the practice-expense RVU, since facility settings (like hospitals) already bear some overhead costs separately from the physician's payment.

RVUs — work 12.68, practice expense 4.04 (non-facility) / 4.04 (facility), malpractice 2.7 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary insurer is responsible for the remainder.
  • Actual payment can vary by locality GPCI values, applicable modifiers, claims-processing adjustments, sequestration, and annual budget-neutrality changes to RVUs or the conversion factor.
  • Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.

Frequently asked

Why does the fee amount change by locality?

Each locality has its own GPCI values for work, practice expense, and malpractice, reflecting regional cost differences. These multiply against the RVUs before the conversion factor is applied, so the same code can price differently across the country.

Why are the facility and non-facility amounts the same for this code?

When the practice-expense RVU is identical for both settings, as shown here, the calculated PFS amount will also be the same regardless of place of service.

Does this amount reflect what the provider actually collects?

No. It reflects the Medicare-allowed amount before the standard 80/20 cost-sharing split, deductible status, and any secondary payer or patient responsibility are applied.

Can the conversion factor or RVUs change mid-year?

The conversion factor and RVUs are typically updated annually, but mid-year adjustments can occur due to legislative action or budget-neutrality corrections, so billers should confirm the current year's figures.
