Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice risk. Each RVU component is adjusted using the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Practice-expense RVUs differ depending on where the service is furnished — non-facility (e.g., physician office) typically has a higher practice-expense RVU than facility (e.g., hospital outpatient), since the facility setting absorbs some overhead costs separately. This is why the same code can show two different national rates, and why your locality-specific rate may differ from both.
RVUs — work 2.1, practice expense 1.31 (non-facility) / 0.65 (facility), malpractice 0.43 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment — actual reimbursement depends on claim-specific factors.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the beneficiary or secondary payer is responsible for the remainder.
- Modifiers, place-of-service codes, multiple-procedure reductions, and sequestration can all change the final paid amount.
- Annual budget-neutrality adjustments and conversion factor updates mean amounts change each year and sometimes mid-year.
- Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.
Frequently asked
Why does the facility rate differ from the non-facility rate for this code?
The facility rate uses a lower practice-expense RVU because Medicare assumes the facility (not the billing physician) is covering certain overhead costs like equipment, supplies, and staff time. The non-facility rate assumes the physician's office bears those costs directly.
Will my locality's payment match the national amount shown here?
Not necessarily. The national amount uses a GPCI value of 1.0 for all components. Your actual locality-specific payment will be adjusted up or down based on that area's wage, rent, and malpractice cost indices.
Can I use this estimate to predict exactly what I'll be paid?
No. This figure is a planning estimate based on the standard PFS formula. Final payment can be affected by modifiers, secondary payer rules, deductible status, claim edits, and annual rate changes.
Where can I find the official CPT descriptor for this code?
CPT code descriptors are copyrighted by the American Medical Association (AMA). You can look up the official descriptor in your licensed CPT codebook or AMA-authorized coding software.
