Medicare Reimbursement for CPT 20701
This page covers Medicare Physician Fee Schedule (PFS) estimated allowed amounts for procedure code 20701. Amounts are shown nationally and can vary by locality, and are broken out by non-facility (e.g., office) and facility (e.g., hospital) settings. Because CPT descriptors are licensed by the AMA, this page refers to it generically as 'this procedure code' rather than reproducing the official description.

Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare PFS amounts are built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to each code. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce a dollar amount. Practice expense RVUs differ between non-facility (e.g., physician office) and facility (e.g., hospital outpatient) settings, which is why the same code can have two different allowed amounts depending on place of service. The conversion factor and GPCIs are updated annually and can change the final amount even if RVUs stay the same.
RVUs — work 1.1, practice expense 0.66 (non-facility) / 0.33 (facility), malpractice 0.21 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary payer is responsible for the remainder.
- Actual payment can be affected by modifiers, place of service, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments.
- Locality-specific GPCIs mean actual allowed amounts will differ from the national figure.
- Always confirm current amounts and coverage rules with your Medicare Administrative Contractor (MAC).
Frequently asked
Why is the facility amount lower than the non-facility amount?
The facility rate uses a lower practice expense RVU because Medicare assumes the facility (not the physician's practice) is bearing most of the overhead costs, such as equipment and supplies, when the service is performed in a hospital or other facility setting.
Will my actual payment match the national amount shown here?
Not necessarily. The national amount is calculated using national GPCI values; your locality's specific GPCIs will adjust the work, practice expense, and malpractice RVUs, which can raise or lower the final allowed amount.
Does this amount include the patient's share?
No. The figures shown represent the Medicare-allowed amount. Medicare generally pays 80% of this after the deductible, with the remaining 20% typically owed by the patient or a secondary payer.
Can modifiers change this amount?
Yes. Modifiers (such as bilateral, reduced services, or multiple procedure modifiers) can increase or decrease the final payment relative to the base PFS amount shown here.
