Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare bases payment on three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice. Each RVU component is adjusted using the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is furnished, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Practice-expense RVUs are typically higher when a service is performed in a non-facility setting (such as a physician office) versus a facility setting (such as a hospital), because the physician absorbs more overhead costs in the office setting. This is why the same code can show two different national or locality-specific allowed amounts depending on place of service.
RVUs — work 1.46, practice expense 0.82 (non-facility) / 0.4 (facility), malpractice 0.26 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary insurer is generally responsible for the remainder.
- Actual payment varies by locality, applicable modifiers, sequestration, and annual budget-neutrality adjustments made by CMS.
- Always verify current rates and coverage policy with your Medicare Administrative Contractor (MAC) before billing.
Frequently asked
Why does this code show two different fee amounts?
Medicare pays differently depending on where the service is performed. The non-facility rate applies to settings like a physician's office, where the practice absorbs overhead costs. The facility rate applies when the service is performed in a hospital or other facility that bears those overhead costs separately.
Why isn't the CPT code description shown here?
CPT code descriptors are copyrighted and licensed by the American Medical Association. This page cannot display or paraphrase the official descriptor, so the code is referenced generically.
Will my locality's payment match the national amount shown?
Not necessarily. The national amount uses a standardized set of inputs, but actual payment is adjusted by locality-specific GPCIs, which can raise or lower the amount compared to the national figure.
Does this amount reflect what the patient will owe?
No. This figure represents the Medicare-allowed amount before cost-sharing. After the Part B deductible is met, Medicare generally pays 80% of the allowed amount, leaving the remaining 20% plus any unmet deductible as patient responsibility, unless covered by supplemental insurance.
