Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's allowed amount for a PFS code is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is performed. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Practice-expense RVUs typically differ between non-facility and facility settings because overhead costs differ, which is why the two settings can produce different payment amounts; for some codes they are the same. Because GPCIs vary by locality, the same code can price differently across the country even though the underlying RVUs stay fixed for the year.

RVUs — work 6.22, practice expense 7.46 (non-facility) / 7.46 (facility), malpractice 1.32 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient's annual deductible is met; the patient or secondary payer is responsible for the remainder.
  • Modifiers, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
  • Amounts shown use national GPCIs and the current year's conversion factor; your Medicare Administrative Contractor (MAC) may apply locality-specific adjustments not reflected here.
  • Always verify current rates and coverage rules with your MAC before billing or appealing a claim.

Frequently asked

Why does the facility amount differ from the non-facility amount?

The practice-expense RVU component is calculated separately for non-facility and facility settings to reflect differing overhead costs. When these RVUs are equal, the allowed amounts in both settings will match; when they differ, the payment amounts will differ.

Will my locality's payment match the national amount shown here?

Not necessarily. The national amount uses standard national GPCI inputs, while actual payment in a specific locality is adjusted using that locality's own GPICs, which can raise or lower the amount compared to the national figure.

Does this page account for sequestration or other payment cuts?

No. The displayed amount reflects the standard PFS calculation before any sequestration reduction, multiple-procedure payment reduction, or other claims-processing adjustment your MAC may apply.

Can I use this amount to predict what my practice will collect?

Use it only as a planning estimate. Actual reimbursement depends on the patient's deductible status, secondary coverage, applied modifiers, and MAC-specific processing rules.
