Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by the Geographic Practice Cost Indices (GPCIs) for a given locality to reflect local cost differences. The adjusted RVUs are summed and multiplied by the annual national conversion factor to produce a dollar amount. Practice-expense RVUs often differ between non-facility (e.g., office) and facility (e.g., hospital) settings, since overhead costs differ; for this code, the practice-expense RVU is the same in both settings, so the non-facility and facility amounts shown here are equal. These RVU values and the conversion factor are updated annually and can change due to policy or budget-neutrality adjustments.
RVUs — work 3.9, practice expense 1.27 (non-facility) / 1.27 (facility), malpractice 1.26 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary insurer is responsible for the rest.
- Actual payment can vary by locality, applicable modifiers, claim-specific adjustments, and federal sequestration.
- Annual conversion factor changes and budget-neutrality adjustments can shift amounts from one year to the next.
- Confirm final payment and coverage policy with your Medicare Administrative Contractor (MAC).
Frequently asked
Why are the non-facility and facility amounts the same for this code?
Because the practice-expense RVU used to calculate this code is identical in both settings, the resulting PFS amounts come out equal. This isn't the case for all codes.
Why can't you show the CPT code description?
CPT codes and their official descriptors are copyrighted by the American Medical Association, so this page refers to the procedure generically rather than reproducing the licensed text.
Will my locality's amount differ from the national amount?
Yes. The national amount uses a national GPCI baseline, while your specific Medicare locality applies its own GPCI values to work, practice expense, and malpractice RVUs, which can raise or lower the final amount.
Does this amount reflect what the patient owes?
No. This figure is the Medicare-allowed amount before cost-sharing; patient responsibility depends on deductible status, coinsurance, and any secondary coverage.
