Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's Physician Fee Schedule assigns each code three Relative Value Units (RVUs): physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual national conversion factor to produce the allowed amount. Non-facility (e.g., office) and facility (e.g., hospital) settings often have different practice-expense RVUs, which is why the same code can show two different allowed amounts. For this code, the non-facility and facility amounts shown are currently the same, meaning the practice-expense RVU component does not differ between settings.
RVUs — work 14.38, practice expense 10.14 (non-facility) / 10.14 (facility), malpractice 3.05 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary payer is responsible for the remainder.
- Modifiers, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
- Locality-adjusted GPCI values and the conversion factor are updated annually and may differ from the figures shown if CMS issues mid-year corrections.
- Always confirm current rates and coverage policy with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.
Frequently asked
Why are the non-facility and facility amounts the same for this code?
When the practice-expense RVU is identical in both settings, the calculated allowed amount comes out the same, even though non-facility and facility rates are generally calculated separately.
Does this amount reflect what my practice will actually be paid?
No. It's a PFS estimate before adjustments for deductible status, secondary insurance, modifiers, or locality-specific corrections your MAC may apply.
Why does the amount change depending on where the code is performed?
Medicare's fee schedule accounts for differences in overhead and resource costs between office/non-facility settings and hospital/facility settings, which is reflected in the practice-expense RVU.
Why can't you show the CPT code description?
CPT code descriptors are copyrighted and licensed by the American Medical Association, so we reference the procedure generically rather than reproducing the official language.
