Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 4 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare PFS amounts are built from three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is furnished, then the adjusted RVUs are summed and multiplied by the annual national conversion factor to produce a dollar amount. Practice-expense RVUs differ between non-facility (e.g., physician office) and facility (e.g., hospital, ASC) settings because overhead costs differ, which is why the same code can show two different allowed amounts. These figures are recalculated annually and can shift due to rule changes even if the RVUs themselves stay the same.
RVUs — work 2.05, practice expense 3.86 (non-facility) / 1.18 (facility), malpractice 0.2 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary payer is responsible for the remainder.
- Actual payment varies by locality, applicable modifiers, place of service, sequestration reductions, and annual budget-neutrality adjustments.
- Always verify the current allowed amount and coverage rules with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.
Frequently asked
Why is the facility amount lower than the non-facility amount?
Facility amounts use lower practice-expense RVUs because the facility (hospital, ASC, etc.) — not the billing physician — absorbs most of the overhead and equipment costs for the service.
Does this amount apply to every locality?
No. The national figure is a baseline; actual payment is adjusted by locality-specific GPCIs, so the real allowed amount can be higher or lower depending on where the service is performed.
Can modifiers change this amount?
Yes. Modifiers such as those indicating bilateral procedures, reduced services, or multiple-procedure reductions can increase or decrease the final payment relative to the base PFS amount shown here.
Why might my remittance advice show a different amount than this page?
Differences can result from sequestration, annual budget-neutrality adjustments, secondary payer coordination, or claim-specific modifiers and edits not reflected in this baseline estimate.
