Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare PFS payment is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — assigned to this code. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce a dollar amount. Practice-expense RVUs differ between non-facility and facility settings because overhead costs (equipment, supplies, staff time) are assumed to be lower when the procedure is performed in a facility that bears some of those costs itself. This is why the same code can show two different national or locality amounts depending on place of service.

RVUs — work 4.67, practice expense 8.1 (non-facility) / 4.09 (facility), malpractice 0.49 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This figure is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary payer is responsible for the remainder.
  • Modifiers, multiple-procedure reductions, bilateral billing, and other claim-specific factors can change the final paid amount.
  • Annual budget-neutrality adjustments, sequestration, and locality-specific GPCI updates can shift amounts year to year.
  • Always confirm current rates and billing rules with your Medicare Administrative Contractor (MAC) before submitting claims.

Frequently asked

Why does the facility amount differ from the non-facility amount for the same code?

The practice-expense RVU component is lower in the facility setting because the facility (such as a hospital) absorbs part of the overhead cost, while in non-facility settings the billing physician's practice bears the full overhead, resulting in a higher practice-expense RVU and payment.

Does this amount reflect what my specific locality will be paid?

The national figures are calculated using national GPCI averages; actual payment in your locality will differ based on that area's specific GPCI values, which are applied in the locality-level amounts shown on this page.

Will the patient's cost-sharing reduce what the practice collects?

Yes — under standard Part B coverage, Medicare pays 80% of the allowed amount after the deductible is met, and the remaining 20% (plus any unmet deductible) is generally the patient's or secondary insurer's responsibility.

Can modifiers change this estimated amount?

Yes, modifiers (e.g., bilateral, multiple procedure, or assistant-at-surgery modifiers) can increase, decrease, or otherwise adjust the allowed amount, and such effects are not reflected in this baseline estimate.
