Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount starts with three Relative Value Units (RVUs) assigned to the code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is billed, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Practice-expense RVUs typically differ between non-facility (e.g., office) and facility (e.g., hospital) settings because overhead costs differ, which is why the two amounts can diverge — for some codes, like this one, they may come out equal. The conversion factor is set annually by CMS and applies uniformly nationwide, while GPCIs vary by locality.
RVUs — work 12.13, practice expense 11.1 (non-facility) / 11.1 (facility), malpractice 2.25 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary insurer is generally responsible for the remainder.
- Actual payment can vary by locality, facility vs. non-facility status, applicable modifiers, sequestration reductions, and annual budget-neutrality adjustments.
- Always verify the final allowed amount and any site-of-service or modifier rules with your Medicare Administrative Contractor (MAC).
Frequently asked
Why are the non-facility and facility amounts the same for this code?
When the practice-expense RVU is identical for both settings, the resulting PFS amount comes out the same for non-facility and facility claims. This isn't universal — many codes have different RVUs (and thus different payments) by setting.
What do work, practice expense, and malpractice RVUs represent?
Work RVUs reflect physician time, skill, and effort; practice expense RVUs reflect overhead costs like staff and supplies (which vary by setting); malpractice RVUs reflect liability insurance costs. All three are summed, geographically adjusted, and converted to a dollar amount.
Why might my locality's reimbursement differ from the national amount shown?
GPCIs adjust each RVU component based on local costs of labor, rent, and malpractice insurance, so the same code can pay more or less depending on where the service is furnished.
Does the conversion factor change every year?
Yes. CMS updates the conversion factor annually, and it can shift due to legislative adjustments, budget-neutrality requirements, and other policy changes, which affects payment for all PFS codes including this one.
