Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount starts with three Relative Value Units (RVUs) assigned to the code: physician work, practice expense (PE), and malpractice (MP). Each RVU component is adjusted by the Geographic Practice Cost Index (GPCI) for the specific locality to reflect local cost differences, then the adjusted RVUs are summed and multiplied by the annual national conversion factor (a dollar-per-RVU rate set by CMS) to produce the allowed amount. Practice-expense RVUs typically differ between non-facility (e.g., physician office) and facility (e.g., hospital, ASC) settings because overhead costs differ; for this code, the non-facility and facility PE RVUs are the same, so the national non-facility and facility amounts shown here are equal. Work and malpractice RVUs do not change by setting.
RVUs — work 12.1, practice expense 11.58 (non-facility) / 11.58 (facility), malpractice 2.25 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the beneficiary or secondary payer is responsible for the remainder.
- Modifiers, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments can change the actual paid amount.
- Locality-specific GPCIs mean the real allowed amount can differ from the national figure shown here.
- Always confirm current-year rates and locality pricing with your Medicare Administrative Contractor (MAC).
Frequently asked
Why are the non-facility and facility amounts the same for this code?
The allowed amount depends on the practice-expense RVU assigned for each setting. For this code, CMS has set the non-facility and facility PE RVUs equal, so the calculated amounts match.
Why might my locality's reimbursement differ from the national amount shown?
The national amount uses a GPCI value of 1.0 for all three components. Actual locality amounts apply that area's specific work, PE, and MP GPCI factors, which can raise or lower the final payment.
Does this amount reflect what the patient or secondary insurer owes?
No. The figure shown is the Medicare-allowed amount. After the deductible is met, Medicare generally pays 80% of this amount, and the remaining 20% (plus any unmet deductible) is the patient's or secondary payer's responsibility.
Will this amount change during the year?
The conversion factor and RVUs are set annually by CMS but can be affected by mid-year legislative or budget-neutrality adjustments, so billers should verify current rates periodically.
