Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

The Medicare PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — that reflect the relative effort and cost of furnishing the service. Each RVU component is adjusted by a Geographic Practice Cost Index (GPCI) specific to the locality where the service is performed, to account for regional cost differences. The adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce a dollar allowed amount. Non-facility and facility rates can differ because the practice-expense RVU is typically lower when the service is performed in a facility (e.g., hospital) setting versus a physician's office; for this code, the non-facility and facility practice-expense RVUs are equal, so the resulting allowed amounts are the same in both settings.

RVUs — work 13.96, practice expense 12.42 (non-facility) / 12.42 (facility), malpractice 2.59 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • All figures are estimates only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the remaining 20% is beneficiary or secondary-payer responsibility.
  • Actual payment can vary by locality, applicable modifiers, multiple-procedure or bilateral adjustments, sequestration, and annual budget-neutrality adjustments to RVUs or the conversion factor.
  • Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.

Frequently asked

Why don't you show the CPT code description?

CPT codes and their official descriptors are copyrighted and licensed by the American Medical Association. This page references the code generically and does not reproduce AMA-owned text.

Why is the facility amount the same as the non-facility amount for this code?

The allowed amount differs between settings only when the practice-expense RVU differs. For CPT 15845, the practice-expense RVU is identical in both non-facility and facility settings, so the calculated amounts match.

Will my locality's payment match the national amount shown here?

Not necessarily. The national figure uses a standard set of geographic adjustors; your actual payment depends on the GPCIs assigned to your specific Medicare locality.

Does this page tell me what my practice will actually collect?

No. This is an estimate based on the published PFS formula. Actual reimbursement depends on deductible status, secondary coverage, modifiers, claim edits, and MAC-specific adjustments.
