Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Each CPT/HCPCS code is assigned three Relative Value Units (RVUs) — for physician work, practice expense, and malpractice risk. These RVUs are each adjusted by Geographic Practice Cost Indices (GPCIs) specific to the Medicare locality where the service is performed, then the adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Non-facility rates apply when the service is performed in a setting like a physician's office, using a higher practice-expense RVU to reflect the overhead the physician incurs; facility rates apply in hospitals or ambulatory surgical centers, where the facility bears most of the overhead and the practice-expense RVU is lower. The conversion factor and RVU values are updated annually by CMS and can shift due to budget-neutrality adjustments.
RVUs — work 8.51, practice expense 15.44 (non-facility) / 7.54 (facility), malpractice 0.84 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, based on national or locality-level PFS data — not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets their annual deductible; the patient or secondary payer is responsible for the remainder.
- Actual payment can be affected by modifiers, multiple-procedure reductions, sequestration, and annual budget-neutrality adjustments.
- Always verify the final allowed amount with your Medicare Administrative Contractor (MAC), as local adjustments may apply.
Frequently asked
Why does the facility amount differ from the non-facility amount for this code?
The non-facility amount includes a higher practice-expense RVU because it assumes the billing practice absorbs overhead costs like staff, supplies, and equipment. The facility amount assumes those costs are borne by the hospital or ASC instead, resulting in a lower allowed amount for the professional service.
Will the amount shown match what I'm actually paid?
Not necessarily. Your actual payment depends on your specific Medicare locality GPCIs, any applicable modifiers, secondary payer rules, and current-year sequestration or other adjustments — all of which can cause the paid amount to differ from the national estimate.
Does this page tell me the CPT code's clinical description?
No. CPT code descriptors are licensed by the American Medical Association and are not reproduced here. Refer to your CPT codebook or EHR for the official descriptor.
Why do Medicare fee schedule amounts change each year?
CMS updates RVUs, GPCIs, and the conversion factor annually, often due to statutory requirements, budget-neutrality adjustments, and periodic revaluation of codes, which can increase or decrease the allowed amount from year to year.
