Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense (PE), and malpractice (MP) — assigned to the code. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the Medicare locality where the service is furnished, to reflect local cost differences. The adjusted RVUs are summed and multiplied by the annual national conversion factor to produce the allowed amount. Non-facility (e.g., office) and facility (e.g., hospital) settings can have different PE RVUs, which is why two rates may be shown; for this code the non-facility and facility PE RVUs are the same, so the resulting amounts match nationally.
RVUs — work 36.22, practice expense 16.32 (non-facility) / 16.32 (facility), malpractice 5.71 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary payer is responsible for the remainder.
- Actual payment can vary by locality GPCIs, applicable modifiers, site of service, sequestration, and annual budget-neutrality adjustments.
- Always verify the current, locality-specific amount with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.
Frequently asked
Why does the amount shown differ from what I see on my MAC's fee schedule?
MAC-published fee schedules reflect locality-specific GPCIs and may include updates or corrections not yet reflected here. Use this page as a general reference and confirm the exact amount with your MAC.
Why are the non-facility and facility amounts the same for this code?
When the practice expense RVU is identical for both settings, the calculated allowed amount is the same regardless of where the service is performed.
Does this amount include the patient's coinsurance?
No. The figure shown is the total Medicare-allowed amount before the standard 80/20 split; Medicare generally pays 80% after the deductible, with the remaining 20% owed by the patient or secondary insurance.
Will this amount change during the year?
It can. Mid-year corrections, sequestration, and annual budget-neutrality adjustments can change the effective payment even if the published RVUs and conversion factor stay the same.
