Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare pricing for this code starts with three Relative Value Units (RVUs): physician work, practice expense, and malpractice. Each RVU component is adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality to reflect local cost differences, then the adjusted RVUs are summed and multiplied by the annual Medicare conversion factor to produce the allowed amount. Practice expense RVUs differ depending on whether the service is performed in a non-facility setting (like a physician's office, where the practice bears more overhead) versus a facility setting (like a hospital, where the facility bears more overhead), which is why non-facility and facility allowed amounts differ. The conversion factor and RVU values are updated annually by CMS.
RVUs — work 4.65, practice expense 12.13 (non-facility) / 5.94 (facility), malpractice 0.87 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets their deductible; the patient or secondary payer is responsible for the remainder.
- Actual payment can vary based on modifiers, place of service, sequestration, and annual budget-neutrality adjustments.
- Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC).
Frequently asked
Why is the facility amount lower than the non-facility amount?
In a facility setting, the facility (not the billing physician) incurs most of the overhead costs, so the practice expense RVU — and therefore the total payment to the billing provider — is lower than when the same service is performed in a non-facility setting.
Does this amount apply to all localities?
No. The national figure uses standardized GPCIs, but actual payment varies by locality because GPCIs adjust for local wage and cost differences. Use the locality-specific figure for more accurate estimates.
Can modifiers change this amount?
Yes. Modifiers such as those indicating bilateral procedures, multiple procedures, or reduced services can increase or decrease the final allowed amount relative to the base rate shown here.
Is this the amount the patient will owe?
Not directly. This is the Medicare-allowed amount before cost-sharing. Patients typically owe 20% coinsurance after the deductible, unless they have supplemental coverage.
