Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) — physician work, practice expense, and malpractice — each adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed. These locality-adjusted RVUs are summed and multiplied by the annual conversion factor to produce the allowed amount. Practice-expense RVUs differ depending on whether the service is reported in a non-facility (e.g., physician office) or facility (e.g., hospital) setting, which is why two different amounts are shown. The conversion factor and RVU values are updated annually by CMS and can change due to budget-neutrality adjustments.
RVUs — work 3.66, practice expense 9.64 (non-facility) / 4.88 (facility), malpractice 0.58 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- All figures are estimates based on the national Medicare Physician Fee Schedule; actual payment may differ.
- Medicare typically pays 80% of the allowed amount after the patient meets their deductible; the patient or secondary insurer is responsible for the remainder.
- Modifiers, place-of-service codes, sequestration, and annual budget-neutrality adjustments can all change the final paid amount.
- Locality-specific GPCIs mean the actual rate in your area may differ from the national average shown here.
- Always verify final payment amounts with your Medicare Administrative Contractor (MAC) before billing or appealing a claim.
Frequently asked
Why is the facility amount lower than the non-facility amount?
The facility rate uses a lower practice-expense RVU because Medicare assumes the facility (e.g., hospital) is incurring overhead costs like equipment and supplies, not the billing physician.
Does this amount reflect what my practice will actually be paid?
No — this is a national or locality-adjusted estimate. Actual payment depends on your specific MAC, contract terms, modifiers used, and any applicable sequestration or policy adjustments.
Why can't you show the CPT code description?
CPT code descriptors are copyrighted and licensed by the American Medical Association, so this page refers to the code generically rather than reproducing the official language.
How often do these amounts change?
CMS updates RVUs and the conversion factor annually, and amounts can also be affected by mid-year legislative or budget-neutrality adjustments.
