Medicare Reimbursement for CPT Code 15002

This page covers a CPT procedure code (descriptor not shown due to AMA licensing) as priced under the Medicare Physician Fee Schedule (PFS). It displays the national Medicare allowed amount as well as how that amount can vary by locality, and separately shows non-facility (e.g., office) versus facility (e.g., hospital) payment rates.

Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare PFS amounts are built from three Relative Value Units (RVUs) assigned to the code: physician work, practice expense (PE), and malpractice (MP). Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the provider's locality to reflect local cost differences. The adjusted RVUs are summed and multiplied by the annual national conversion factor to produce the allowed amount. Because practice expense differs depending on where the service is performed, non-facility (e.g., physician office) and facility (e.g., hospital outpatient) settings use different PE RVUs, resulting in two different payment amounts for the same code.

RVUs — work 3.56, practice expense 6.66 (non-facility) / 1.69 (facility), malpractice 0.65 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary payer is responsible for the remainder.
  • Actual payment depends on the provider's specific locality GPCIs, which can differ from national averages shown here.
  • Modifiers, place-of-service codes, multiple-procedure reductions, and bilateral/assistant-surgery rules can change the final paid amount.
  • Annual budget-neutrality adjustments, sequestration, and mid-year rule changes can affect the conversion factor and final payment.
  • Always verify the exact allowed amount with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.

Frequently asked

Why does the facility amount differ from the non-facility amount for this code?

The practice expense RVU is lower in the facility setting because Medicare assumes the facility (not the physician's practice) is absorbing certain overhead costs, such as equipment and supplies, when the service is performed there.

Does this amount reflect what Medicare will actually pay my practice?

No — it reflects the national PFS allowed amount before locality adjustment. Your actual Medicare payment will be based on your specific locality's GPCIs and any applicable modifiers or adjustments.

Why isn't the CPT code description shown here?

CPT codes and their descriptors are copyrighted and licensed by the American Medical Association (AMA), so this page references the code generically rather than reproducing the official descriptor.

How often do these RVUs and the conversion factor change?

CMS updates RVUs and the conversion factor annually (and sometimes mid-year) through rulemaking, so amounts can change from one year to the next even if the code itself stays the same.
