Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Each CPT/HCPCS code is assigned Relative Value Units (RVUs) for physician work, practice expense (PE), and malpractice (MP). These RVUs are each adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is performed, then summed and multiplied by the annual Medicare Conversion Factor to produce a dollar amount. Practice expense RVUs differ depending on whether the service is reported in a non-facility setting (e.g., physician office) or a facility setting (e.g., hospital outpatient), which is why two different allowed amounts are shown. The conversion factor and GPCIs are updated annually and can shift payment amounts even if RVUs stay constant.

RVUs — work 11.19, practice expense 17.36 (non-facility) / 8.4 (facility), malpractice 1.33 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary payer is responsible for the remainder.
  • Modifiers, multiple-procedure reductions, bilateral adjustments, and site-of-service differences can change the actual paid amount.
  • Annual budget-neutrality adjustments and sequestration may further reduce the amount actually disbursed.
  • Always verify current, locality-specific rates with your Medicare Administrative Contractor (MAC) before billing or appealing a denial.

Frequently asked

Why does the facility amount differ from the non-facility amount for this code?

The practice expense RVU component is lower in the facility setting because Medicare assumes the facility (not the physician's office) is bearing certain overhead costs, such as equipment and supplies.

Does this amount reflect what Medicare will actually pay?

No. It reflects the calculated PFS allowed amount before the 80/20 coinsurance split, deductible status, and any claim-specific adjustments like modifiers or sequestration are applied.

Why might my locality's amount differ from the national figure?

Medicare adjusts RVUs using locality-specific GPCIs to account for regional differences in wages, rent, and malpractice costs, so the same code can pay differently across geographic areas.

Where can I find the official descriptor for CPT 14061?

CPT descriptors are copyrighted by the American Medical Association. Check your AMA CPT codebook, encoder software, or MAC fee schedule lookup tool for the official language.
