Maximum units per day (MUE)
Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.
How the amount is calculated
Medicare's PFS amount is built from three Relative Value Units (RVUs) assigned to the code: physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the service locality, then the adjusted RVUs are summed and multiplied by the annual conversion factor to produce a dollar amount. Non-facility and facility rates differ mainly in the practice-expense RVU, since facility settings (hospitals, ASCs) already bear some overhead costs that are instead billed by the facility itself. The national figures shown here use a standard GPCI baseline; applying your specific locality's GPCIs will produce the locality-adjusted amount.
RVUs — work 9, practice expense 13.04 (non-facility) / 7.1 (facility), malpractice 1.07 — × the locality GPCIs × the 2026 conversion factor of $33.40.
Good to know
- This is an estimate only, not a guarantee of payment.
- Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the patient or secondary insurer is responsible for the remainder.
- Modifiers (e.g., bilateral, multiple procedure, assistant-at-surgery), sequestration, and annual budget-neutrality adjustments can change the final paid amount.
- Locality-specific GPCIs can meaningfully shift the amount up or down from the national figure shown.
- Always verify current rates and coverage rules with your Medicare Administrative Contractor (MAC) before billing.
Frequently asked
Why is the facility amount lower than the non-facility amount?
The facility rate uses a lower practice-expense RVU because the facility (hospital, ASC, etc.) is separately reimbursed for overhead such as equipment, supplies, and staff time, whereas in a non-facility setting the physician's practice absorbs those costs.
Does this amount reflect what my practice will actually be paid?
No. It is a national estimate before locality GPCI adjustment, the 80% Medicare payment share, deductible status, modifiers, and any sequestration or annual rate updates are applied.
Why isn't the CPT code description shown here?
CPT code descriptors are copyrighted and licensed by the American Medical Association (AMA), so this page cannot reproduce the official language and instead refers to the code generically.
How often do these amounts change?
The conversion factor, RVUs, and GPCIs are updated annually (and sometimes mid-year) by CMS, so the estimated amount for this code can change from year to year.
