Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 1 unit per patient per day — Date of Service Edit: Policy. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's PFS amount is built from three Relative Value Units (RVUs): physician work, practice expense, and malpractice. Each RVU component is adjusted by Geographic Practice Cost Indices (GPCIs) specific to the locality where the service is performed, reflecting regional cost differences. The adjusted RVUs are summed and multiplied by the annual conversion factor (a dollar-per-RVU rate set by CMS) to produce the allowed amount. Non-facility rates (e.g., office settings) use a higher practice-expense RVU than facility rates (e.g., hospital settings), since the provider incurs more overhead costs in non-facility settings. This is why the same code often has two different national or locality-adjusted amounts.

RVUs — work 4.23, practice expense 7.82 (non-facility) / 2.25 (facility), malpractice 0.5 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only — actual payment depends on the specific locality, claim details, and Medicare Administrative Contractor (MAC) processing.
  • Medicare typically pays 80% of the allowed amount after the annual Part B deductible is met; the patient or secondary insurer is responsible for the remainder.
  • Modifiers, multiple-procedure reductions, and other claim-specific adjustments can change the final paid amount.
  • Amounts are subject to sequestration reductions and annual budget-neutrality adjustments made by CMS.
  • Always verify current rates and local policies with your MAC before billing or appealing a denial.

Frequently asked

Why is the facility amount lower than the non-facility amount for the same code?

Facility rates use a lower practice-expense RVU because the hospital or facility — not the billing provider — bears most of the overhead and supply costs for the service.

Does this amount apply everywhere in the country?

No. The national amount is a baseline; actual payment is adjusted using locality-specific GPCIs, so the real allowed amount can be higher or lower depending on where the service was performed.

Can I use this figure to predict an exact payment on a claim?

No. This is an estimate based on standard PFS inputs. Final payment depends on additional factors such as modifiers, secondary payer coordination, and claim-specific adjustments applied by the MAC.
