Maximum units per day (MUE)

Medicare’s Medically Unlikely Edit caps this code at 2 units per patient per day — Date of Service Edit: Clinical. Units above the MUE are denied; some MUEs can be exceeded only with appropriate documentation and modifiers where policy allows.

How the amount is calculated

Medicare's PFS payment is built from three Relative Value Units (RVUs) assigned to a code: physician work, practice expense, and malpractice. Each RVU component is adjusted by the Geographic Practice Cost Indices (GPCIs) for the specific locality where the service is furnished, since costs vary by region. The adjusted RVUs are summed and multiplied by the annual conversion factor (a dollar figure set each year) to produce the allowed amount. Non-facility and facility rates differ mainly because the practice-expense RVU is higher when the provider bears the overhead (e.g., in an office) versus when the service is performed in a facility that separately bills its own overhead.

RVUs — work 2.01, practice expense 3.97 (non-facility) / 2.02 (facility), malpractice 0.27 — × the locality GPCIs × the 2026 conversion factor of $33.40.

Good to know

  • This is an estimate only, not a guarantee of payment or coverage.
  • Medicare typically pays 80% of the allowed amount after the patient meets the annual Part B deductible; the beneficiary or secondary payer is responsible for the remainder.
  • Actual payment can vary by locality, applicable modifiers, sequestration reductions, and annual budget-neutrality adjustments to the conversion factor.
  • Always verify the final allowed amount and payment with your Medicare Administrative Contractor (MAC), as local pricing files may differ from these estimates.

Frequently asked

Why does the facility amount differ from the non-facility amount?

The facility rate uses a lower practice-expense RVU because the facility (e.g., hospital) — not the billing physician — incurs and bills separately for the overhead costs of the encounter.

Does this amount reflect what Medicare will actually pay?

No. It reflects the PFS-allowed amount before the standard 80/20 split, deductible status, modifiers, and any sequestration or local adjustments are applied.

Why does the fee change year to year?

CMS updates RVUs, GPCIs, and the conversion factor annually, and these updates — along with statutory budget-neutrality requirements — can raise or lower the allowed amount from one year to the next.

Can I use this figure to bill a specific locality?

Use the locality-adjusted figure shown on this page as a reference estimate, but confirm the exact amount with your MAC's published fee schedule, since GPCI values are locality-specific.
